    Complete this form if your school is interested in receiving a Farm to School Culinary Kit for the 2026-27 school year. Submission of this form does not guarantee that your school will receive a kit. Culinary Kits will be distributed on a first-come, first-served basis, with priority given to schools that do not already have a Culinary Kit. The person completing this form will serve as the primary contact. Kits are intended to be shared across K-5 classrooms in your district.
